CUSTOMIZED PARTY ROOM FORM

Please send me a E-mail with all the information below according to what you would like on your VP Customized Party Room. If anything is left out then the form will be returned. Filling out this form does not make you liable for purchasing any thing; it is just to receive a price and to make sure I can do what you would like done.

After filling out the form and sending I will send you back a confirmation E-mail along with a price of what your page will cost.

If you decide to purchase the page then send a check or money order along with any pictures or music you want used (unless you are going to E-mail it or want me to find it for you); sorry at the present time I do not except credit cards. Once I receive your money your page will be posted.

You should allow one week from the time your money is received for your page to be posted for your confirmation; although it don't usually take that long. This is also as long as you respond to ALL of my E-mails immediately. Till your page is done please check your E-mail at least daily and respond immediately so that I can get your page done on time.

Your page will be hosted on Angelfire; page should be used for VP purposes only; you will have a angelfire URL unless you decide on the findhere URL; the room will not be linked to without your permission; your name will be posted as a reference for pages I have done Customized VP Party Room for; my small logo linking to my page will be posted in the bottom corner of your page; the room will belong to you although your room should only be used as a Customized VP Party Room; any original items sent through the mail will be returned to you, no guestbooks can be added to these pages since they are not websites; and only I will have the password and ID for the pages. Sorry, I can not turn the passwords or ID's over to you either; they are used for me to change anything needed. If you would like anything changed in the future; I can do it for a additional charge.

Anything I make for you from scratch, you will get three different ones in your email to choose from. When your page is done you will be notified by E-mail that your page is ready for your approval; are able to look at it and if anything is wrong with it then you get a chance to have me change it for you. You will be given one week to respond if you want anything changed.
